Commissioners for taking affidavits take affidavits or declarations by asking you to swear or affirm what is in a document is true.

Notary publics are regulated under the Notaries Act and can verify that signatures, marks and copies of documents are true or genuine. Notaries also have the authority to administer oaths, solemn affirmations, and declarations that are used for affidavits and statutory declarations.

Every lawyer in Ontario is automatically a commissioner for taking affidavits. 

When do you need an affidavit?

  • In court, for example, small claims, family and/or civil court;
  •  Transferring ownership of a used vehicle to another person;
  • Gifting ownership of a used vehicle to a family member (Sworn Statement);
  • Applying for a municipal licence or an exemption to a zoning restriction
  • Consent to travel documents (Sample Consent to Travel form);
  • Common-law statutory declarations;
  •  Affidavit for single parent OSAP application

  • Bring one (1) government issued photo identification or two government issued non-photo. identifications. Health cards and SIN cards are not acceptable.

For Certifying/Notarizing True Copies

  • Bring the original document to be photocopied at the office. The Notary must view the original.
  • Multiple single documents must be photocopied individually, and each image requires its own seal.
  • Multi-page documents require only one (1) seal. The Notary will seal the first page and emboss the other pages.
  • Complete all the documents before arriving at our office.
  • DO NOT sign or date the documents before meeting the Notary. It is the Notary’s duty to properly identify you and witness you signing.
